All Arts for All participants must have the following 3 forms on file:

1. Medical Release

2. Photo & Media Release

3. Handbook Form

There are two options for completing all the forms:

1. Forms may be completed online and submitted from the website (online option).

2. Forms may be downloaded, completed, and then submitted online through this webpage (download option).
